Access to your health information is important. My Health Portal is an easy way for staff to electronically share your health information with you – and you can view it at your convenience, anywhere from an internet-enabled device.

With My Health Portal you are able to:

  • View your health records

  • View your lab, x-ray, and other test results

  • Store important health information

Not signed up yet?

  1. Let the front desk team know you’re interested in signing up.
  2. The front desk will give you a PIN to use to complete the registration process.
  3. Visit login.intelichart.com or download the Intelichart app.
  4. Complete the registration process.
  5. Once registration is completed, you will receive a verification link via email.

This is a free service to offer you more convenience and access to your health records.
